New Patient Registration
If you live within our practice area (please check first using the Postcode Checker on our Practice Boundary Map page) you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so it would help if you could provide proof by way of a recent utility bill.
You will need to complete a registration form (GMS1, and bring a form of ID with you) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. You may print off a registration form, fill it out and bring it in with you on your first visit to the practice. We will also ask you to consent to share your medical record so that other health professionals can be fully informed when consulting with you. This includes your Summary Care Record with Additional Information. Please see below link.
Medical treatment is available from the date of registration. Please contact reception for further information.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking. If you are on any repeat medication, you will need to make an appointment with a Doctor so they can reissue your medication appropriately.
You may also like to complete a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
