Travel Health Questionnaire

We need this questionnaire completing FOUR WEEKS BEFORE your travel date in order to get your vaccinations in stock and an appointment booked - AFTER TEN WORKING DAYS FROM SUBMITTING YOUR TRAVEL QUESTIONNAIRE, PLEASE PHONE THE SURGERY TO ASK WHAT VACCINATIONS/MEDICATION ARE REQUIRED (IF ANY) AND TO BOOK YOUR TRAVEL APPOINTMENT.

Travel Questionnaire

If you require any vaccinations relating to foreign travel you need to complete the above questionnaire and bring this into the Surgery so that a Practice Nurse can decide what, if any, vaccinations or medication you may need for your travel. IT IS IMPORTANT THIS IS COMPLETED PRIOR TO ANY APPOINTMENT BEING MADE.

Travel Vaccination Advice

Some vaccines have to be ordered as they are not a stock vaccine. Your appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
